The Physical Therapist PRN plays a critical role in delivering high-quality rehabilitative care to patients within their home environments, ensuring personalized treatment plans that promote recovery and enhance mobility. This position focuses on assessing patient needs, developing and implementing therapeutic interventions, and monitoring progress to achieve optimal functional outcomes. The role requires adaptability to varied home settings and collaboration with interdisciplinary teams to coordinate comprehensive care.
As a Physical Therapist, you will:
Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.
Applies concepts of infection control/universal precautions in performing patient care
Assure continuity of quality patient care delivered with appropriate documentation
Provide effective communication to patient/family, team members and other health care professionals
Promote agency philosophy and administrative policies
Maintain and complete all documentation within company stated guidelines.
Requirements
Successful completion of accredited physical therapy program and state licensure exam.
Valid Physical Therapy license
1 year of home health experience preferred
CPR certification
Valid Driver’s license
Valid Auto Insurance
HomeCare HomeBase experience preferred
