Job Summary:

The Hospice Director of Support Services oversees spiritual care, bereavement care and volunteer programs across multiple locations. This role develops and coordinates chaplaincy programs, provides spiritual and emotional support to patients and families, and recruits, trains, and supervises volunteers. The Director acts as a liaison between the interdisciplinary care team, volunteers, and patients, ensuring documentation, program development, and quality assurance for spiritual, bereavement, and volunteer services.

Qualifications


Qualifications:

  • Minimum of one year in spiritual care, preferably in a healthcare setting and/or with terminally ill patients.
  • Thorough knowledge and understanding of all aspects of home health and hospice services
  • Master’s degree in divinity, Counseling, Social Work, Healthcare Administration, or related field preferred.
  • Professional or academic background in chaplaincy, pastoral care, hospice, or palliative care.
  • Strong interpersonal and communication skills, crisis intervention, knowledge of end-of-life issues, and collaborative work style.
  • Proficiency with documentation and basic computer skills. 
  • Ability to travel in various forms of transportation with overnight stays
  • Ability to read, write, and speak the English Language
  • Highly developed management, communication and interpersonal skills
  • Ability to evaluate, revise, and implement systems

 

Case Manager Essential Functions:

  • Program Leadership: Supervise and support chaplains and volunteers, foster a compassionate environment and ensuring quality of care and compliance with organizational standards.
  • Spiritual Care: Provide or oversee direct spiritual care, including counseling, grief support, and end-of-life rituals for patients and families.
  • Program Management: Develop, implement, and manage spiritual care programs aligned with patient and family needs and organizational values.
  • Volunteer Management: Recruit, train, and supervise volunteer chaplains and other volunteers for meaningful engagement. Participate in interdisciplinary team (IDT) meetings to integrate spiritual and volunteer components into patient care plans.
  • Bereavement Program Leadership: Develop, implement, and evaluate bereavement care programs for patients’ families, ensuring compliance with regulatory and organizational standards. Supervise bereavement coordinators and staff, providing guidance, training, and support. Oversee the delivery of grief counseling, support groups, memorial services, and follow-up care for bereaved families.
  • Interdisciplinary Collaboration: Serve as a liaison between the interdisciplinary care team, patients, families, and volunteers.
  • Administrative Duties: Maintain records, document services and interactions, participate in patient care conferences, and ensure compliance with organizational standards and ethical guidelines.
  • Documentation and Compliance: Ensure accurate and timely documentation of services, meeting regulatory standards and organizational policies.
  • Community Coordination: Establish and maintain relationships with community clergy and spiritual leaders to coordinate care and facilitate services requested by patients and families.
  • Quality Assurance: Ensure the quality of spiritual care, bereavement program and volunteer services through record review and program development.
  • Education and Training: Provide education on spiritual care topics to staff, volunteers, and the community.  Provide education on grief and bereavement topics to staff, volunteers, and the community.
