  • 1.0 FTE (40 hours/week)
  Both are full-time, benefits-eligible, and come with a guaranteed salary.

• Primary Responsibilities

  Provide Physical Therapy to home care patients and case management when indicated. Provide timely and accurate communication and documentation relative to physical therapy services. Provide clinical supervision for the Physical Therapy Assistant. Promote and develop physical therapy services in homecare/hospice.

💰 Compensation & Benefits

• Salary: $93,000 - $99,000

Experience:
💙 2+ years of Home Care experience preferred — we love PTs who know how rewarding patient-centered care can be!

Requirements:
✅ Graduate of an accredited Physical Therapy School
✅ Current PT License in the State of Minnesota, in good standing
✅ Current CPR Certification for Health Care Providers
✅ Valid Driver’s License — you’ll be bringing quality care right to your patients’ homes
