Full-Time: Monday-Friday 8am-4:30pm
SUMMARY
Responsible for programmatic leadership for community-based programs and services including but not limited to: Health Home, Non-Medicaid Case Management (Jefferson and Lewis Counties only), Supportive Housing, HUD Continuum of Care funded projects, SPOA (Lewis County only), and AOT (Lewis County only).
May be required to use personal vehicle with mileage reimbursement.
ESSENTIAL DUTIES AND RESPONSIBILITIES
- Oversees the development of quality individual service plans and the establishment, review and maintenance of client care protocols ensuring that all state policies, guidelines and regulations are met for all clients accessing Thrive services. Ensures client confidentiality is maintained at all times.
- Provide guidance in times of crisis and is available to community-based staff for guidance outside normal business hours.
- Responsible for the resolution of personnel issues. Complete staff evaluations in accordance with agency policies. Maintain time cards and attendance of staff. Interview, hire, reward and discipline staff in conjunction with the Senior Director of Community Operations, as needed. Participate in the development of staff and identify their training needs as required.
- Maintain regular staff contact to promote the philosophies of the agency; facilitate team meetings to ensure that all necessary program information, training and agency policies and procedures are reviewed; conduct monthly supervisions of assigned staff.
- Work closely with the Senior Director of Community Operations and the Assistant Director of Housing in the fiscal management and development of budgets for assigned services to review profit and loss analysis and work with staff to ensure spending is monitored and adhered to.
- Participate in community meetings such as Priorities Council, SPOA, Health Home, Supportive Housing, HUD and other additional meetings as assigned; represent the agency at community events and promote new or existing agency programs/policies.
- In conjunction with the Senior Director of Community Operations, Assistant Director of Housing and Corporate Compliance, review annual satisfaction surveys and progress towards established goals and objectives for assigned Thrive programs, and participate in developing a plan for ongoing improvement of services.
- Responsible for Compliance and Quality Assurance audits as outlined in the Quality Improvement Program and assigned by the Program Director.
- Review monthly billing logs and check for accuracy of documentation attesting to the billable core services provided in the Health Home and Supportive Housing programs.
- Lead Coordinator of reporting systems including but not limited to MAPP, Foothold, eCR, HeC, CAIRS and other reporting systems as assigned; collect data from these systems to complete reports and/or grants for additional funding as assigned or requested.
- May assist Directors in the development of services and special projects as needed/requested.
- May cover short-term absences of staff for consistency of services to clients. Assist in covering the absence of the Community-Based Manager in another county, if needed.
- Develops and maintains good community relationships and maintains client confidentiality at all times.
- Keep abreast of recent developments, legislation, and any other information affecting Behavioral/Physical Health and Housing services on a local, regional, state, and national level, and share that information with appropriate staff.
- Functions as a member of the multi-disciplinary team.
- Lewis County only: Responsible for SPOA and AOT services including staff supervision, making sure reports are completed for the DCS and that services are compliant with required OMH policies and procedures.
- Other related duties as assigned.

EDUCATION and/or EXPERIENCE
Required:
Bachelor’s degree in a Human Service Field and Two years’ experience, or NYS licensure and current registration as an RN with a bachelor’s degree and two years' experience or a credentialed Alcoholism and Substance Abuse
Counselor (CASAC) and two years' experience or bachelor’s level education or higher in any field with five years of experience.
CERTIFICATES, LICENSES, REGISTRATIONS
Must maintain a valid driver’s license and acceptable driving record.
