Under the direction of the VP, the Director plans, implements and directs services and programs at the Pathways Center for Comprehensive Care (DAC HIV Service). This includes directing daily operation and managing staff, alongside operational leadership, direction, and oversight of the Routine HIV Screening and Undetectables programs across Emergency, Inpatient, and Ambulatory Care settings.
The role directs clinical workflow development, Epic EMR enhancements, workforce training, quality improvement, and data reporting. As a member of the department’s leadership team, the position holder collaborates with stakeholders to drive program excellence and aligns initiatives with SBH health system's goals, promoting innovative and effective patient-centered care.
Responsibilities:
- Provides leadership to the Ambulatory Care Quality & Population Health department, supporting the VP with department strategy, operations and management.
- Directs operations and services at the Pathways Center for Comprehensive Care (DAC HIV Service).
- Direct program development to strengthen and grow Pathways services.
- Lead program integration efforts in SBH Health System to strengthen inward referrals and ensure Pathways patients connect with appropriate SBH resources including Health & Wellness, Population Health, Health Home, care management, other special programs, and community resources.
- Maximize visit volume by overseeing daily operations activities with scheduling, registration, call reminders, and follow-up of missed visits.
- Directs the development and implementation of population health programs to improve patient care and operational performance, including the Routine HIV Screening and Undetectables programs.
- Collaborates with clinical and administrative leaders to direct clinical workflow development, Epic EMR enhancements, workforce training, quality improvement, and data reporting.
- Manages and supervises assigned employees / teams, including work assignments, attendance, discipline, performance evaluations and planning requirements.
- Ensures departmental compliance with all NYCDOHMH and NYSDOHAI regulations, standards, and guidelines.
- Respond to staff and patient concerns as they arise to mitigate any potential undesirable situations.
- Participates in the wider departmental leadership team, leading change across the department and its divisions.
- Performs other related duties at the discretion of the Vice President.
- Attends scheduled department and division meetings and participates as appropriate.
