About Us:

Western Reserve Area Agency on Aging (WRAAA) is a private non-profit corporation, organized and designated by the State of Ohio to be the planning, coordinating and administrative agency for federal and state aging programs in Cuyahoga, Geauga, Lake, Lorain and Medina Counties. It is one of twelve Area Agencies on Aging (AAAs) in the state organized together with local service provider organizations and the Ohio Department of Aging (ODA) to form the state's public aging services network. The network collaborates together to create opportunities for older Ohioans to receive needed home and community services and support; and to age successfully in their own homes and communities.

Position Overview:

This position is responsible for both managing a caseload (50% of the role) and leading a small team (50% of the role). The Supervisor ensures smooth transitions of care, enhances well-being, and fosters independence for members. In addition to their clinical responsibilities, the Supervisor provides leadership, strategic direction, and support to the team, ensuring that all program goals are met efficiently and with high-quality standards.

 

Responsibilities:

  • Supervise and support the assigned programs by offering leadership, mentorship, and opportunities for professional growth to boost staff performance and job satisfaction.
  • Ensure smooth implementation of care transition processes by fostering collaboration between members, healthcare providers, and the agency, while maintaining excellent service standards.
  • Monitor program activities to ensure adherence to agency policies, contract regulations, and professional standards. Implement initiatives to improve service quality.
  • Promote effective communication among team members and stakeholders to ensure continuous and consistent care delivery, while addressing and resolving any issues related to member care and service coordination.
  • Ensure accurate data entry and upkeep of member records, including admissions, discharges, and transitions, while safeguarding data integrity and security.
  • Efficiently manage resources to optimize their use, meeting departmental objectives and supporting staff requirements.
  • Conduct training sessions and participate in educational opportunities to stay informed on industry trends and best practices in care transition management.
  • Conduct home visits and telephonic interventions as needed to fulfill program and individual quotas.
  • Provide backup for staff and assist with handling workload as necessary.
  • Perform chart audits and offer regular feedback to staff.
  • Perform additional duties as assigned.

 


Qualifications:

  • An active Registered Nurse (RN) or Licensed Social Worked (LSW) in the state of Ohio.
  • RN is preferred
  • A minimum of 3 years of proven experience in a managerial or supervisory role within a clinical or healthcare environment preferred.
  • Deep knowledge of healthcare regulations, compliance standards, and quality assurance practices.
  • Exceptional leadership, communication, and interpersonal skills.
  • Competency in data analysis, problem-solving, and decision-making.
  • The ability to multitask, set priorities, and effectively manage time in a fast-paced setting.
  • Able to travel to five county territory (Medina, Geauga, Lake, Lorain, and Cuyahoga)
