The Altenheim is now hiring a Transitional Care Nurse (RN or LPN). We offer daily and weekly pay options and great benefits!
The purpose of the Transitional Care Nurse is to focus on the management of chronic disease and conditions within the facility to reduce hospital readmission rates in accordance with accepted standards of practice, state and federal regulations, and operational policies and procedures. We are an Indiana-based and family-owned provider of senior living options and lifestyle support services, ranging from independent living and assisted living to skilled nursing and memory care. EDUCATION, QUALIFICATIONS, CREDENTIALS: Must be a Licensed Nurse within the State of Indiana; LPN preferred. Experience in a hospital setting is acceptable, long term care and/or case management experience, preferred. Certification as a Case Manager, preferred.
RESPONSIBLE TO: Health Facility Administrator, or Director of Nursing.
