Work Schedule: Monday - Friday 8:30 am - 5:00 pm (Some on-call responsibilities)

Since 1971, the Hampton-Newport News Community Services Board has provided a comprehensive continuum of high-quality, essential, and community-based services to citizens of the Peninsula who are living with mental illness, developmental disabilities, and substance use disorders.
Case Management is a vital part of the H-NNCSB service delivery system, serving as a unifying link between families, children, and the supports they need. The Case Manager acts as a single point of contact for families, providing continuous assessment, coordination, and monitoring of services for children and adolescents experiencing serious emotional disturbances or substance use disorders. Through interagency collaboration, planning, and treatment oversight, the Case Manager helps families access social, educational, vocational, and community resources. Intensive Care Coordination (ICC) offers a higher level of support, including clinical assessments and level-of-care determinations, for individuals diagnosed with a Serious Emotional Disturbance (SED) or an Intellectual or Developmental Disability (IDD).
Your day-to-day as a Child & Adolescent Intensive Care Coordination Case Manager
As an Intensive Care Coordination Case Manager, you will play a key role in supporting children with Serious Emotional Disturbance (SED) or Intellectual and Developmental Disabilities (IDD). In this vital position, you will conduct assessments, develop individualized service plans, and coordinate care across multiple systems to ensure each child receives the comprehensive support they need. Your work will involve building strong relationships with children, families, and community partners, advocating for appropriate services, and responding to crises with professionalism and compassion. Collaborating with a dedicated multidisciplinary team, you will develop creative care plans tailored to each child's unique needs. Each day, you will witness the positive impact of your efforts and take pride in knowing that your work is truly changing lives for the better.
What we are looking for
To thrive in this role, you'll need:
A Bachelor's degree in a Human Services, such as Social Work, Psychology, Counseling
Two (2) years of experience providing mental health case management or care coordination services for children
Knowledge of DSM V diagnosing and care management principles
A valid driver's license
