POSITION SUMMARY: The Schedule Navigator is responsible for providing a variety of office support functions, including but not limited to: scheduling and coordinating office visits, making and receiving telephone calls, filing, and data entry. The position provides administrative support to the clinic staff, providers, therapies departments and external contracted providers, while ensuring all participant appointments are scheduled, tracked, and reported to all relevant stakeholders.

RESPONSIBILITIES AND DUTIES:
ESSENTIAL JOB FUNCTIONS:
- Schedules and coordinates internal and external clinic appointments to include but not limited to: Rocky Mountain Health Care Services (RMHCS) clinic, clinic assessments, PT/OT, dental, dermatology, orthotic, vision clinic, and other contracted providers appointments
- Collaborates with specialists and required RMHCS disciplines to ensure appropriate treatment plan coordination
- Calls to participants, MDPOA, Facilities and CareCom’s when making clinic and outside appointments
- Coordinates scheduled appointments with the transportation department
- Documents all incoming and outgoing calls in the participants’ chart
- Enters data into the Electronic Medical Record (i.e., scheduling, verification of services, etc.).
- Creates and maintains documents which may require arithmetic calculations
- Assists in the care and maintenance of department equipment and supplies
- Completes all other duties as assigned

Qualifications
- High school diploma or GED equivalent
- 2 years of office related experience
- 1-year experience working with the frail or elderly preferred
- Electronic medical records experience preferred
- Knowledge of medical terminology preferred
- Knowledge of standard medical office practices and procedures including filing, record keeping, and the use of standard office equipment such as computer and copy reproduction equipment
- Computer proficiency in office software and procedures with ability to maintain accurate records, compile data, and prepare reports.
