According to established criteria, reviews patient medical records to abstract and assess data. Identifies and assesses quality and patient care performance through clinical analysis in a confidential manner. Plans, organizes and directs clinical, performance improvement, research, education and recertification of the Medical Center's Stroke Program. Ensures compliance with all appropriate safety and infection control standards. Coordinates quality assessment and continuous quality improvement activities. Performs all job duties and responsibilities in a courteous and customer-focused manner according to the Hurley Family Standards of Behavior and Management Code of Conduct. Works under the Senior Administrator - Nursing or designee who reviews work performance primarily through conferences and oral and written reports. Works under the supervision of the departmental director or designee who may review and inspect work for effectiveness and conformance to established standards and procedures. May exercise complete supervision over employees in assigned areas and units. Will have responsibility for Purchasing Agents and MMIS staff and functions required of that area of responsibility. May also be required, at the discretion of the Director, to act as lead to contracting and purchasing activities should the Director be unavailable.

Responsibilities

  1. Formulates, recommends, and implements systems, policies, and procedures to ensure compliance with standards of care, regulations, quality of care and patient outcomes.
  2. Coordinates and implements cost-reduction strategies while optimizing clinical quality of care.
  3. Leads quality improvement initiative teams. Develops meeting agenda, sets timelines, and monitors team progress toward goals.
  4. Ensures optimal coordinated clinical care for patients across continuum through concurrent and retrospective performance improvement process including oversight review of management guideline adherences, audit filters, deaths, and complications.
  5. Participates in development, implementation, and accomplishment of departmental goals, objectives and research relative to quality and patient safety activities. 
  6. Ensures registry data integrity, data validation, and compliance with data submission requirements 
  7. Reviews data for patterns and trends as well as presents reports based upon data collected to appropriate committees and leaders for performance improvement, research, education, financial, and programmatic purposes.
  8. Participates in ongoing design and maintenance of the stroke registries by communicating with stroke registry vendors and hospital information technology department to ensure technical support including responsibility for registry upgrades, training, and maintenance of database.
  9. Participates in quality and patient safety rounding and facilitates follow-up through written and oral communications specific to quality and patient safety concerns and issues.
  10. Coordinates or provides stroke education and Stroke Outreach Programs to pre-hospital, nursing and physicians to improve care.
  11. Promotes professional nursing and by serving as host preceptor for visiting program managers, nursing students, and other allied health students.
  12. Participates in resident, fellow, physician assistant, nursing, and allied health personnel orientations that pertain to quality and patient safety activities.
  13. Promotes Hurley stroke program externally to increase public knowledge of disease and strategies to decrease likelihood of death and disability.
  14. Performs other related duties as required. Utilizes new improvements and/or technology that relate to job assignment.

Qualifications

  • Bachelor of Science in any Healthcare related field.
  • Master of Science in any Healthcare related field preferred.
  • Three (3) years of nursing experience in related specialty area.
  • Knowledge and skill with Epic.
  • Knowledge of basic computer applications such as word pocessing, spreadsheets, and media.
  • Must possess a high level of verbal and written communication skills.
  • Ability to collect and analyze data and present same in written or oral formats.
  • Ability to maintain harmonious relationships with patients, their families, the public, nursing personnel, other health care professionals, and other medical center staff.

NECESSARY SPECIAL QUALIFICATIONS:

  • Licensed to practice as a registered nurse in the State of Michigan.
  • Stroke Certified Registered Nurse (SCRN) or Certified Critical Care Nurse (CCRN), or other national certification in an appropriate specialty area preferred.
  • Strong evidence of local, state, or national contributions to the field of stroke care preferred.
  • Must provide evidence of completion of eighteen (18) hours of stroke related continuing education per year of which six (6) hours must be accredited by a professional nursing organization.
