Summary

Under the direction of the Director of Nursing Education and Professional Practice, this individual will coordinate and supervise the overall function of the Stroke Program at Concord Hospital. This will require working collaboratively with physicians, nurses and ancillary personnel. The Stroke Program includes the continuum of care from pre-hospital, acute care, rehab and follow-up.

Education

Bachelor's of Nursing Required. MSN Preferred

Certification, Registration & Licensure

Certification required: BLS Licensure and NIHSS certification required: Licensed to practice Nursing in the State of New Hampshire. Stroke Certified Registered Nurse from the American Association of Neuroscience Nursing (SCRN) required.

Experience

Minimum of 4 years of acute care nursing. Previous supervisory/leadership experience desired. Program Management or QA experience preferred.

Responsibilities

  • Provides support, coordination, and oversight for the Stroke Program.

  • Coordinates the continuum of stroke patient through put.

  • Provides consultation and education to staff.

  • Manages Human, Material and Fiscal Resources for the Stroke Program.

  • Oversees and manages Performance Improvement activities for the Stroke Program.

  • Provides Clinical Coordination and participates in direct care for stroke patients.
