Provides skilled nursing assessments of client/caregivers in the Homecare setting which includes physical, psychosocial and environmental assessment and care. Develops a care plan in consultation with the client /family unit, the interdisciplinary team, and the primary physician. Coordinates and plans with the professionals and caregivers involved, to meet the goals of the client’s plan of care. Supervision of LPNs and HHAs.
Work Schedule: Casual; 32 hours a month plus weekend and holiday rotation
Required Qualifications:
- Associate’s nursing degree from an accredited school
 - Must have MN or WI RN license upon start date.
 - Must provide proof of MN and WI RN license within 4 months of start, if both MN and WI RN licenses are not present on start date.
 - Must have a dependable vehicle, valid driver’s license, auto insurance and meet all criteria required under Lakeview’s Automobile Liability Guidelines
 - Knowledge of reimbursement procedures as well as knowledge of community resources
 - Accept responsibility for maintaining current knowledge level of nursing trends in the Hospice field.
 - Able to function independently with minimal supervision in home setting
 - Strong assessment skills
 - Ability to read, write and speak the English language
 - Ability to carry out directions
 - Computer experience
 - Comfortable in end-of-life care
 - Flexible with changing schedule
 - Willing to participate in On-Call rotating schedule
 - Bachelor’s in nursing from an accredited school
 - Master’s in nursing from an accredited school
 - One year experience of Homecare, Hospice, or Long-Term Care
