Role and Responsibilities

Clinical:

· Rooming clients, vitals, ht/wt

· Assessment/Screenings (COWS, CIWA-Ar, ACSA, etc.)

· Medication administration & tracking

· Provide medication education to clients

· Maintain medical equipment in proper working order; maintain confidentiality and proper boundaries

· Obtain urine drug screens, HCG screens or other screens as indicated; Lab to pick up when indicated

· Venipuncture for baseline labs and labs as needed; Lab to pick up

Care Coordination and Clinic Duties:

· Order and maintain clinic supplies

· Complete prior authorization forms for Insurance, Medicaid, and Medicare Part D recipients

· Run KASPER reports

· Assist with referral process, copying, filing, updating MR, client consents & edu packets

· Coordinate with IDT (CM, peer support, other programs) for continuity of care

· Other duties as assigned

Qualifications

Qualifications and Education Requirements

· Registered Nurse with unrestricted license in Kentucky; At least 1 year of experience preferred

· Special Requirements

· Good Health

· Must be able to lift at least 50 lbs.
