Position Responsibilities:

  • Acts as a liaison between patient and CCC infusion team to ensure timely delivery of care; reports delays to Team Leader and/or nurse assigned to patient, and documents patient interactions in the EMR. 
  • Works in collaboration with the Team Leader and the Pre-Treatment RN in reviewing weekly infusion schedules to ensure accurate patient appointments according to patient's treatment plan.
  • Identifies barriers that hinder effective care of patients receiving new and scheduled treatment cycles.
  • Reviews infusion schedules with focus on unlinked appointments to ensure established guidelines are followed and as directed by the Team Leader.
  • Communicates with patient and staff to ensure understanding of next appointments for lab, physician, and treatment and confirms appointments are scheduled prior to patient's discharge.
  • Assists in referring patients to the multidisciplinary team, i.e. patient educator for new treatment patients, social workers, and clinical dietitians, as needs are identified from patient interactions/rounding, and as directed by nursing staff or team leader, nurse manager, or director.
  • Coordinates care of patients transitioning from inpatient to outpatient, working with Team Leader to ensure appropriate consents are signed and patient education is completed.
  • Informs/calls patients when scheduled appointments have to be moved to a different day, time, or cancelled appointments, as directed and based on the Pre-Treatment RN's review of treatment or supportive plans.
  • Coordinates scheduling of patients from clinics outside of the CCC, i.e. GI, Alzheimer's Clinic, Urology, etc., to ensure patients are given appointments according to established scheduling guidelines; prints copies of orders, attaches labels on forms, hands to TL for review, and once patient completes treatment, documents sent to HIM Department for scanning.
  • Coordinates with the clinical teams with the goal of improving patient's adherence to treatment plans, patient safety, increased involvement in care, patient satisfaction, and optimal levels of health and well-being of patients and overall survival.
  • Facilitates efficiencies in departmental processes and workflows, patient flow into the infusion suite, ensuring patient safety and improve patient satisfaction.
  • Participates in staff meeting, quality improvement initiatives, department projects

     

Qualifications:

  • OPN-Certified Generalist preferred, or obtain within 3 months of hire
  • Associate's Degree in related discipline preferred.                                                                                   
  • 3-5 years experience in patient/healthcare relations or human services field preferred, or combination of education and experience. Training in health and community outreach programs preferred but not required. Experience in care coordination and referral to internal and external resources preferred.
