The Connected Care Navigator at Boston Children's Hospital plays a critical role in delivering white glove, patient-centered care to diverse patient populations with a focus on those seeking virtual care. This role focuses on supporting Patients needing complex care coordination, including those referred from specialists nationwide or undergoing highly specialized diagnostic evaluations, by coordinating appointments, reducing barriers to care such as insurance and transportation, and facilitating access to digital health resources. The Navigator collaborates closely with interpreters, clinical teams, and administrative staff across multiple locations to ensure seamless delivery of care.
Key Responsibilities:
- Provide high-touch, personalized support to patients and families throughout their care journey. Identify and address barriers to care, including insurance coverage, transportation challenges, and language access.
- Facilitate smooth transition of care after the initial virtual visit, facilitating a seamless hand-off between the Connected Care Hub and foundation clinics. Collaborate with interpreters and care teams to ensure culturally competent communication. Learn and maintain an appointment template, build access to support scheduling workflows, and ensure accurate, efficient coordination across departments.
- Serve as the primary point of contact for incoming patients and their families, providing a welcoming and professional first impression. Conduct thorough patient intake processes to ensure all necessary details, including registration and digital onboarding, are accurately and efficiently collected. Maintain accurate documentation and perform administrative duties across multiple BCH sites.
- Coordinate with previous healthcare providers to collect necessary medical records and documentation, ensuring all documents are properly uploaded and organized. Schedule initial virtual consultations with specialists or advanced practice providers according to patients’ needs and provider availability. Maintain organized scheduling systems to ensure efficient workflow.
- Educate patients about the virtual visit process, including the necessary technology requirements and expectations for the visit. Provide technical assistance to patients as needed to access telehealth services. Work closely with medical providers, administrative staff, and IT support to ensure a seamless patient experience.
- Develop and maintain training materials to support patient education and staff onboarding. Track tasks and progress using Jira or similar project management tools and provide weekly report-outs to relevant stakeholders. Draft internal and external communications related to digital health initiatives, patient outreach, and program updates.
- Open the digital front door for all patients by activating MyChart accounts, educating patients on its features, and supporting digital onboarding to enhance engagement and access.
Education:
- Bachelor’s degree or equivalent experience required.
- 3+ years in a healthcare environment required.
- Experience in patient navigation, care coordination, or community health preferred.
