Health Navigation Specialist - Job Overview

Key Responsibilities

  • Implement health education programming in both one-on-one and group settings.
  • Utilize motivational interviewing and goal-setting techniques to meet patient needs.
  • Provide referrals and assist with linkage to services to enhance patient independence.
  • Conduct point-of-care testing for HIV and other health conditions.
  • Contribute to the development of new health education programs.
  • Collaborate on an interdisciplinary clinic and care team to foster effective working relationships.
  • Conduct assessments of patient needs and track their progress.
  • Establish referral processes and build community networks.
  • Promote access to culturally sensitive care in a timely manner.
  • Document patient services in the electronic health record with accuracy.

Minimum Qualifications

  • High School Diploma or equivalent is required.
  • 3-5 years of relevant experience in a related field is mandatory.
  • Bachelor’s Degree in a related field with at least 1 year of relevant experience is preferred.
  • Experience in performing point-of-care testing and educating diverse populations.
  • Ability to demonstrate HIV care and prevention competencies within 3 months of hire.
  • Phlebotomy training and bilingual proficiency in English/Spanish is preferred.
  • Proficiency in Word, Excel, and other computer programs.
