Location: In-person work required in the Triangle Area (Wake, Durham, Orange Counties), NC
The Autism Society of North Carolina is hiring a full-time Care Manager to support individuals with autism and developmental disabilities through compassionate, community-based care. What You’ll Do:
- Provide whole-person care management for individuals and families
- Collaborate with care teams, service providers, and families
- Educate clients on assessments, care plans, and available services
- Conduct assessments and develop personalized Plans of Care
- Submit referrals, manage authorizations, and monitor service delivery
- Ensure compliance with internal and external standards
What We’re Looking For:
- Bachelor’s or master's in human services (Social Work, Psychology, etc.)
- 2+ years' experience with Autism and/or Intellectual /Developmental Disabilities
- Based in or near the Triangle area and willing to travel locally
- Strong communication, organization, and problem-solving skills
- Self-starter who thrives in a collaborative environment

Qualifications
Minimum Qualifications:
- Bachelor's degree from four-year college or university in a discipline related to human services, and a minimum of two years of full-time, post baccalaureate, accumulated experience working with persons with intellectual/developmental disabilities; or Bachelors degree in a field unrelated to human services and a minimum of four years of full-time, post baccalaureate accumulated experience working with persons with intellectual/developmental disabilities; or Masters degree in a human services field and a minimum of one year of full-time, post-graduate, accumulated experience working with persons with intellectual/developmental disabilities
- Two years’ experience providing Care Management, Case Management, or Coordination to complex individuals with IDD/ASD
- Ability to work independently from a satellite office covering a region of several counties without an onsite supervisor
- Valid driver’s license and reliable transportation
- Preferred Spanish speaking
