Description

Lifeworks is hiring a full-time Consumer Directed Community Supports (CDCS) Service Coordinator to join our In-Home Services team. In this role, you will provide case management and personalized support to individuals with disabilities and their families. You’ll help clients manage their FMS budgets, navigate Minnesota waiver programs, coordinate services, and ensure their funding aligns with their goals and priorities.


As a CDCS Service Coordinator, you’ll work with a high level of autonomy while being part of a collaborative Self-Directed Services team. This position requires strong organizational skills, a deep understanding of relevant regulations, and a passion for empowering individuals to lead self-directed lives.


Position Title: Consumer Directed Community Supports (CDCS) 

Department: Self-Directed and In-Home Services

Hours: Full-time 40hours/week, Monday – Friday (exempt)

Location: Lifeworks Services – Richfield / 20% in-person

Compensation: $49,500 - $55,500

Bonus Eligible: Yearly

  • Thorough understanding of disability services and funding sources available in Minnesota highly preferred.
  • Travel in the greater metro area when necessary is required.
  • Must pass a background study with the Department of Human Services, a drug test, clean driving record and have reliable transportation to use for work driving purposes (e.g., in-office, on-site training locations, etc.).

Requirements

  • Two-year degree in human services or related field with two years of direct care to individuals with disabilities or individuals of 65 years of age or older.
  • Demonstrated interpersonal skills that quickly build rapport with customers, county social workers, and administrative staff.
  • Case management experience related to self-directed services preferred.
  • Candidate must complete and pass a background study, pre-employment drug screen; have a valid driver’s license free of violation; proof of motor vehicle insurance and reliable transportation for work purposes.
