Job Description:

· Collaborate with clients, families, and stakeholders to develop and implement individualized treatment plans
· Utilize evidence-based practices, such as Motivational Interviewing, Positive Reinforcement Techniques, and CBT/DBT approaches
· Coordinate care with other service providers, including primary care physicians and community organizations
· Maintain accurate and timely records of client interactions and progress
· Bachelor's degree in unrelated field with two years of related work experience; or
· Any four-year combination of higher education and two years of related work experience
· Current driver's license, acceptable driving record, and current auto insurance
· Ability to work with information in a confidential manner
· Highly detail-oriented, skilled in accuracy
· Organization skill set and ability to make contacts and establish relationships
