Job Summary

The Patient Flow Center is intended to improve the quality and safety of care, reduce the need for out of area placements, and optimize the use of resources. The Manager provides leadership, management, and administration of all aspects of the Patient Flow Center (PFC) in order to ensure organizational mission and objectives are met. This role typically reports to the Director of Operations for the Patient Flow Center.

Job Description

The Patient Flow Center is seeking an experienced RN to join their team! This Manager role will oversee the Patient Flow Center with approximately 41 direct reports. This position is located at Memorial Hermann Tower - Memorial City.

- Must have BSN

- Must have 10 or more years of RN experience in Patient Placement, OA, Patient Flow, bedding, or throughput

- Must have 5 or more years of supervisory experience

- On call rotation: 1 week every 6 weeks

- Work Hours: Mon-Fri/8-5 (hours may vary)

MINIMUM QUALIFICATIONS

Education:  Bachelors of Nursing and Graduate of an accredited School of Professional Nursing required.

Licenses/Certifications:  

  • Current State of Texas license or temporary/compact license to practice professional nursing required

  • Additional certifications may be required based on discipline and/or nursing unit requirements

Experience / Knowledge / Skills:

  • Five (5) years of clinical experience required; Two (2) years supervisory experience required.

  • Previous patient placement/transfer center experience preferred

  • Effective oral and written communication skills.

  • Strong interpersonal skills and ability to work effectively at all levels in a collaborative team environment.

  • Knowledge of the principles and practices of hospital and clinical operations and/or healthcare management.

PRINCIPAL ACCOUNTABILITIES

  • Implement and manage processes that will enhance access to Memorial Hermann Healthcare, services, physicians, and health care professionals

  • Assist with hiring and maintaining appropriate staffing levels and utilization of personnel and ensure the highest quality of professional skill

  • Provide a structured means for policy and procedure development through regularly scheduled staff meetings allowing for continual evaluation, review, and update of standards, functions, goals, and objectives

  • Respond promptly to changes in workload, adjusting assignments as necessary to achieve optimal productivity and efficiency

  • Provides input to annual and interim performance appraisal reviews for the professional and non-professional staff in department.

  • Responsible for leading a high performance team of “system thinkers” who incorporate leadership principles and vision in performing the functions of case management.

  • Create and manage schedules for the PFC team members

  • Collaborate with other departments to ensure efficient operations

  • Participate in quality assurance and improvements of the transfer process and documentation, developing appropriate follow up plans

  • Responsible for new hire orientation, ISD access and time & attendance entry/oversight.

  • Function as both manager and PFC agent when delegated as such

  • Maintain team member performance scorecards

  • Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.

  • Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.

  • Demonstrates commitment to caring for every member of our community by creating compassionate and personalized experiences. Models Memorial Hermann’s service standards of providing safe, caring, personalized and efficient experiences to patients and our workforce.
