We are seeking a compassionate and motivated
Part-Time YDT Case Manager to join our Youth Diversion Team. This grant-funded role focuses on supporting youth involved in diversion programming by providing case management, mentorship, and behavioral support.

Position Duties


  • Attend weekly (as identified) Youth Diversion Team (YDT) panel hearings.
  • Conduct weekly, biweekly, or monthly check-ins with assigned YDT youth.
  • Attend Planning & Placement Team (PPT)or other school-based meetings as designated.
  • Provide support, guidance, and feedback to youth participants.
  • Attend summer camp daily and provide behavioral support to youth, counselors, and supervisors.
  • Complete all required documentation and data entry in accordance with Crisis Intervention program standards.
  • Participate in mandatory supervision meetings and monthly staff meetings.
  • Attend trainings and conferences as designated by the CID program supervisor.
    • Provide case management and regular check-ins for youth referred to the YDT program.
    • Ensure youth complete their assigned dispositions and connect them to appropriate services.
    • Offer mentorship and guidance to youth participants.
    • Deliver daily behavioral support during the East Hartford Parks & Recreation summer camp.
    • Collaborate with schools, Youth & Social Services, Police Social Work, Youth Outreach, and Parks & Rec to support youth success.
    • Participate in community outreach and education events, occasionally during evenings or weekends.

Minimum Qualifications

  • Excellent time management, flexibility in scheduling, and ability to multi-task effectively.
  • Experience working with youth with mental health and co-occurring disorders, and their families or support systems.
  • Strong understanding of the juvenile justice system and recovery-based approaches.
  • Ability to work independently and as a collaborative team player with town departments, schools, and juvenile court.
  • Demonstrated cultural competency and cultural humility, especially in diverse communities.
  • Awareness and sensitivity to diversity, equity, and inclusion in client and staff interactions.
  • Ability to maintain confidentiality and ensure the security of client records.
  • Strong documentation skills and ability to incorporate recovery-oriented language into daily operations.
East Hartford is a richly diverse community, with approximately 70% of the population identifying as persons of color. The ideal candidate will bring a strong understanding of cultural responsiveness and a commitment to equity and inclusion in service delivery.

Other Qualifications

  • Bachelor’s Degree with a minimum of 5 years’ experience in human services, criminal justice, education, social work, or youth/adult rehabilitative services. 

  • Must have a valid driver’s license.
    • Bilingual/Bicultural (Spanish/English) skills are helpful but not required.
    • Previous experience with juvenile justice systems (e.g., youth probation) is preferred.
    • Familiarity with the Town of East Hartford and local community resources is a plus.
