Full Time | Monday–Friday, 8:00 AM – 4:30 PM

Overview:
The McGuire Group is seeking a dedicated Admissions Screener to serve as the frontline liaison between hospital discharge planners, referral sources, and our long-term care and sub-acute rehabilitation facilities. This is a hospital-based role, focused on screening patients for appropriate placement into our skilled nursing communities across Western New York.

Key Responsibilities:

  • Serve as the point of contact for hospital discharge planners, social workers, and case managers.

  • Review medical records and patient information to determine appropriate placement within our facilities.

  • Conduct medical, clinical, and social assessments for prospective admissions.

  • Communicate with McGuire Group admissions coordinators, nursing leadership, and insurance authorization teams to facilitate timely transitions.

  • Coordinate patient transfers and transportation logistics when needed.

  • Maintain accurate documentation of referrals, screenings, and outcomes in tracking systems.

  • Build and maintain positive relationships with hospital staff and internal facility teams.

Requirements:

  • Healthcare background required; experience in admissions, case management, discharge planning, or related roles strongly preferred.

  • Knowledge of long-term care and sub-acute rehab placement criteria.

  • Strong communication, organizational, and interpersonal skills.

  • Familiarity with EMR systems and referral portals (e.g., Epic, PCC a plus.)

  • Must be able to travel between hospital and facility sites as needed.
