Job Summary

This position serves as a role to connect families with community resources to help maintain and preserve families. The Family Support Navigator will work primarily in the 32206 zip code serving families at Center of Hope’s (COH)Kennedy Center location.   

 

This position requires direct contact and consultation with clients and is intended to ensure maximum utilization of resources for these individuals. The Family Support Navigator will report to the Executive Director of Community Reinvestment. This role uses strong understanding and knowledge of resources available in the community to stabilize families.

Job Duties

  • Assess needs of families/patrons with SDOH assessment tool and refer patrons to COH’s partner network and community providers in a timely manner..
  • Provide follow up services to COH patrons (when needed and appropriate) to ensure that crisis has been stabilized
  • Provide crisis intervention linkages and referrals through consultation to ensure family support and stabilization.
  • Assist with the facilitation of parent and community support groups, workshops, and focus groups
  • Provide emergency assistance which includes delivering or purchasing items to families such as food, beds, clothing and assisting with emergency housing; hotel and housing payments; prepare financial requests to assist families in need.
  • Participate in utilization reviews, tracking, and management of service provision and demographics information to help identify trends and potential service gaps, community needs.
  • Upload data in CoBris on a daily basis to ensure program and funding compliance.
  • Serve as task supervisor for social work and psychology student interns.
  • Attend community resource meetings, as requested and needed

 

Minimum Qualifications

  • Bachelor’s Degree in Sociology, Social Work, Psychology or Human Services from an accredited college or university is required. Master’s degree in a Human Services field or area of study from an accredited college or university can substitute for one year of experience. 
  • At least three years of case management and service coordination experience working with at-risk children and/or families with significant mental health and behavioral needs. Must be familiar with community resources in the 32206-zip code. Must have an understanding of child development and risk factors that may lead to child abuse and neglect.

 

  • Valid Driver’s License

 

 

Schedule & Hours

Typically Monday-Friday, 9-5PM and possible evenings & weekends
