At Project Transition, it's our mission to enable individual persons who have serious mental illness, co-occurring substance use disorder and/or a dual diagnoses of SMI and IDD live a life that is meaningful to her or him in the community on terms she/he defines.

Summary Job Description:

Provide social work and case management services for members, families and other providers/funding sources to ensure coordination of care for assigned members.

Specific Responsibilities:

  • Coordinate new admissions to program with Admissions and Program staff
  • Complete Incident Reports following process indicated by Manager Care Organization and PT's P&P
  • Participate in the treatment/recovery planning process with program and staff members
  • Provide documented case management services to members and families including:
  • Coordination of financial eligibility and benefits
  • Coordination of family contact and support
  • Conduct utilization and member care reviews with MCO’s and other payers
  • Oversee Level of Care changes and documented aftercare planning with members
  • Communicate with both referral services and hospitals to facilitate member return to program in a well-coordinated manner
  • Consult with families, psychiatrists, therapists for assigned members
  • Prepare and provide group services at assigned program
  • Participate in phone coaching/crisis management system which includes:
  • Orient member and family to phone coaching system
  • Carry and respond to coaching phone calls on assigned shifts
  • Attend Team meetings, consultation team and staff meetings
  • Back-Up Support to Program Director (coverage)
  • Complete continuing educational requirements as aligned with regulatory compliance
  • Other duties as assigned by Program Director

Qualifications:

  • A graduate of a college or university with a Master's degree in a Human Service field (Preferred) with at least one (1) year of full-time, post-graduate degree accumulated experience with the population served, or a substance abuse professional with one (1) year of full-time, post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling.
    Two years’ experience in case management, financial eligibility, family support and collaboration of care 
  • Demonstrated ability to work in a collaborative team model. 
  • Strong assessment and documentation skills
