Apostolic Christian Skylines has an opening for an Admissions Coordinator. This is a full-time position and will include coordinating all admissions within the Nursing Center and Memory Care Unit. It will also include other nurse administrative duties that may include assisting with the Resident Assessment / MDS process. Candidate must be highly motivated, personable, and dependable. Computer skills and people skills are very important.


Job Responsibilities:

  • Maintain set occupancy level for Skilled Nursing & Memory Care.
  • Network with hospital transition specialists, social workers and other referral sources.
  • Conduct tours and have phone conversations with potential new residents and families.
  • Access resident’s clinical and behavioral needs.
  • Identify Medicare and/or other insurance sources, obtain insurance authorization and complete Medicare admission forms with resident / resident representative as necessary.
  • Coordinate the admissions process including preadmission screening, background checks, PASRR screening, application and admission paperwork completion.
  • Welcome and orientate the new resident and family to Skylines.
  • Decide and coordinate room changes within the facility.
  • Coordinate discharge planning working with community providers.
  • Perform and enter resident assessments as part of the MDS process

7. Ability to hear and speak well, which allows for effective oral communication of information

8. Professional appearance, dress, and mannerism, physical and emotional stamina to effectively handle job-related issues.
