SUMMARY: The behavioral health consultant is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health consultant is responsible for supporting and coordinating the whole patient’s care, substance use, mental and physical health care of patients with the patient’s medical provider and, when appropriate, other behavioral health providers. This position will also focus on ensuring individuals identified with a substance use and/or mental health disorder receive treatment, prevention and awareness of opioid use.
PRIMARY DUTIES and RESPONSIBILITIES include the following: (other duties may be assigned)
ADMINISTRATIVE RESPONSIBILITIES include the following: (other duties may be assigned)
- Support the mental and physical health care of patients based on needs identified by MedNorth Care Team. Closely coordinate care with the patient’s medical and behavioral health providers and, when appropriate, other agencies such as DSS, APS, housing or others.
- Provide same-day crisis assessment and intervention for patients presenting at MedNorth Health Center for services.
- Provide assessment for readiness for MAT services and support for patients receiving MAT.
- Work collaboratively with MAT team.
- Facilitate referrals for clinically indicated services outside of the organization (e.g., social services such as housing assistance, vocational rehabilitation, mental health specialty care, substance use treatment), and facilitate patient engagement and access to resources and follow-up care.
- Provide patient education about common mental health and substance use disorders and the available treatment options. Provide education on opioid use to patients.
- Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
- Support medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
- Provide behavioral health assessments and therapeutic interventions (therapy) to an assigned patient panel using evidence-based techniques such as solution focused therapy, Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, trauma-informed interventions, motivational interviewing, or other treatments as appropriate.
- Participate in regularly scheduled caseload consultations with members of patient care team.
- Track patient follow up and clinical outcomes.
- Document patient progress and treatment recommendations in EHR and other required systems to be shared with medical providers, psychiatric consultant, and other treating providers. Include all phone and face to face encounters.
- Create and maintain treatment plans as appropriate for patient diagnosis and treatment needs in collaboration with the medical provider and the psychiatric consultant.
- Participate in community collaboratives and meetings.

SUPERVISORY RESPONSIBILITIES: N/A
EDUCATION AND/OR EXPERIENCE:
- Master’s degree in social work or other related field
- LCSW or LCSW-A or similar clinical licensure in the state of North Carolina
- LCAS or LCAS-A or significant experience working with individuals with substance use disorder.
- Bi-lingual Spanish speaking preferred but not required
QUALIFICATION REQUIREMENTS: The requirements below are representative of the knowledge, skills, and/or ability required:
- Demonstrated ability to collaborate and communicate effectively in a team setting.
- Ability to maintain effective and professional relationships with patients and other members of the care team.
- Experience with screening for common mental health and/or substance abuse disorders.
- Working knowledge of differential diagnosis of common mental health and/or substance abuse disorders, when appropriate.
- Ability to effectively engage patients in a therapeutic relationship, when appropriate.
- Ability to work with patients by telephone as well as in person.
- Experience with assessment and treatment planning for common mental health and/or substance use disorders.
- Working knowledge of evidence-based psychosocial treatments and brief behavioral interventions for common mental health disorders, when appropriate (e.g., motivational interviewing, problem-solving treatment, behavioral activation).
- Basic knowledge of psychopharmacology for common mental health and substance abuse disorders that is within appropriate scope of practice for type of provider filling role.
- Knowledge of community resources and outlets for patients
ESSENTIAL TECHNICAL/MOTOR SKILLS: Medical terminology, accurate keyboarding skills, ability to process work error free - Ability to use standard office equipment, including computers, photocopiers, and facsimile machines -- Ability to learn, understand, and use Internet insurance eligibility systems.
INTERPERSONAL SKILLS: Excellent interpersonal and customer service skills - Excellent telephone etiquette - Teamwork and communication skills essential -- Treat all patients, co-workers, and managers with courtesy and respect.

LANGUAGE SKILLS: Ability to speak English fluently -- Spanish speaking or another language is a plus.
LICENSES, CERTIFICATES, REGISTRATIONS: LCSW-A or LCSW or similar clinical licensure in the state of North Carolina.

MedNorth Health Center provides comprehensive health care services, inside a multi-cultural medical/dental clinic, to all individuals; insured, uninsured, and underinsured. The building is heated in winter and air conditioned in the summer with noise levels of moderate to loud. We serve patients who speak multiple languages and who may need the assistance of an interpreter. In this position one may be subject to exposure to copier toner, correction fluid, bright fluorescent lighting, and continuous personal computer exposure.
This job is a bloodborne pathogens risk category III position.
