Responsibilities:

 

  • Primary responsibility to work in collaboration with the Admissions Intake Management team and the Admissions Intake Coordinators to ensure all referrals are properly processed and scheduled timely and appropriately to facilitate a smooth transition to the hospice program, while improving speed and accuracy to meet operational key objectives are met.
  • Appropriately assigns Admissions Nurse and Care Partner visits with regard to time and geographic location. Works under direction Admissions Management team for scheduling appointments and consistently informs Admissions Operations Manager of referral and scheduling activities.
  • Ability to handle high volume of incoming calls and demonstrates excellent customer service and public relations skills to all clients, customers
  • Assists the Admissions Nurse by following the patient throughout the referral-intake-admission process to facilitate a smooth transition to the hospice program  Processes the referral, collects all data possible from referral sources to ensure efficient, pre-qualifying of patients.
Qualifications:

 

  • Associates degree preferred.
  • Must have 3 years of experience in healthcare administrative or customer service industry.
  • Knowledge of hospice and home health programs is desirable.
  • Organized, flexible, self-motivated with the ability to communicate with all levels of the medical team.
  • Ability to develop HPBC clinical knowledge base.
  • Computer literate.
  • Excellent communication skills, both verbal and written. Bilingual Preferred.
  • Ability to establish effective relationships with both internal and external customers.
