Regionalized Call Center, Triage LPN

Valley Health Partners, Allentown, PA- Remote/hybrid possible

Triages incoming calls in an ambulatory setting from patients, makes clinical decisions based on protocols, and communicates with supporting clinical manager, and providers.  Provides telephone advise and decision support services.  Identifies clinical escalation and provides effective resolutions.

 

Requirements: 

Education:                                                                  

  •  Graduate of an accredited Nursing program and licensed in the state of Pennsylvania

 

Experience:                                   

  • One year experience as a Licensed Practical Nurse.
  • Three-five years’ experience as a Licensed Practical Nurse in a pediatric setting, School Based Health Program, or Federally Qualified Health Center, preferred.
  • 1 year of call center triage experience, preferred.

 

Licensure:                                     

  •  Active/current  Licensed Practical Nurse license and Basic Life Support (BLS) required.

 

Skills needed:

  • Ability to initiate, accept, and adapt to change within a healthcare setting.
  • Ability to work with culturally diverse people is a must.
  • Excellent time management skills.
  • Excellent computer skills including EMR, word processing, and Excel.
  • Knowledge of registration, insurance, managed care plans, medical authorization requirements, and referral coordination.

Essential Functions:

 

Performs all aspects of patient care, including but not limited to patient assessment, medication administration.

 

  • Provides or delegates care according to VHPCHC policies.
  • Implements individualized patient care program within the health center.

 

Performs patient care and procedures utilizing sound clinical judgment and approved policies/ procedures/protocols/techniques, equipment, and supplies.

 

  • Participates in clinical meetings and other multi-disciplinary quality service improvement initiatives.
  • Stays abreast of new trends and innovations in the areas of disease prevention and treatment, chronic diseases.
  • Compiles and submits necessary medical/statistical reports and data to internal departments or external partners in a timely fashion and, if applicable, as required by law.
  • Actively demonstrates a working knowledge of nursing theory, techniques, principles, and practice in order to holistically care for the patient.

 

Assists and instructs in patient and family teaching, health education and promotion and disease prevention.

  •   Acts as a resource for patient and family members, listens effectively, and gives and receives constructive feedback.
  • Provides high quality care over the phone.
  • Assists patients in navigating the health care system. Coordinates specialty care, follow-up on test results and other care coordination needs.

 Utilizes appropriate and acceptable intervention behaviors and communication styles in order to effectively facilitate teamwork.

  •  Complies with monitoring and documentation of daily activities and expenditures as per standards set forth by accrediting bodies.
  • Participates in quality improvement and infection control initiatives and activities.
