Medical Center: Community Health Development (Full-Time, Day Shift) -

 

Job Summary:  The Community Health Worker-Violence Intervention Professional (VIP) is a vital member of the Loma Linda University Health Hospital Based Violence Intervention Program. The VIP is on the front lines everyday connecting with individuals and family members presenting to Loma Linda University Medical Center who are at highest risk of perpetrating violence or being victimized or affected by violence. Every client in the Violence Intervention Program is paired with a VIP who assists them with identifying personal goals, mapping out an action plan to reach those goals and carrying out the necessary steps to achieving those tasks. Connects with clients while still in the hospital and works with the client to gain their trust. Serves as a role model for the client, acts as an advocate on their behalf, and provides crisis intervention when needed to support the client. Works daily with the inpatient Trauma Team and other healthcare providers, providing updates on the client's status, which ensures that the team is doing everything possible to support the client in reaching his/her goals. Spends 70% of their time in the community which includes home visits and 30% of their time in the hospital connecting with new clients and continuing building relationships with existing clients. Performs other duties as needed.

Education and Experience:  One year experience working in a community based setting preferred.

Knowledge and Skills:  Knowledge usually acquired through related field or acceptable experience in violence intervention, prevention, substance abuse, mental health disorders, and/or development of programs designed to support the community and reduce violence. Ability to display professional demeanor and has excellent communication skills, written and verbal, to relate to people of varying socioeconomic class with different lifestyles or cultural values and to interact with community-based service organization professionals. Working knowledge with crisis interventions strategies. Strong time management and organization skills to maintain project tracking activities and manage multiple priorities. Strong analytical ability to assess various crisis situations and recommend appropriate plan of care Ability to work independently with limited supervision. Computer skills in spreadsheets, word processing, database and graphics in order to enter and retrieve related information for effective reporting and correspondence. Possess flexibility and adaptability to take on new skills and knowledge to meet project needs.

Licensures and Certifications: Valid Driver's License Required at time of hire.   Successful completion of a Community Health Worker formal training program, such as from a college of other education institution is preferred.
