Program Description:
Enhanced Care Management (ECM) is a program within a far-reaching, multiyear plan to transform California's Medi-Cal system, known as California Advancing and Innovating Medi-Cal (CalAIM). Led by California's Department of Health Care Services, the goal of CalAIM is to help integrate Medi-Cal more seamlessly with other social services and help improve outcomes for the millions of Californians covered by Medi-Cal, especially those with the most complex needs. Customers enrolled in the ECM program receive comprehensive care management from a single System Navigator who coordinates all their health and health-related care, including physical, mental, and dental care, and social services. ECM makes it easier for customers to get the right care at the right time in the right setting and receive comprehensive care that goes beyond the doctor’s office or hospital. Who We Serve 

Pacific Clinics serves children, transitional-age youth, families, adults, and older adults. We offer a full range of mental and behavioral health services, foster care and social services, housing, continuing adult education and early childhood education programs to Medi-Cal-eligible individuals and families throughout Alameda, Contra Costa, Fresno, Kings, Los Angeles, Madera, Orange, Placer, Riverside, Sacramento, San Bernardino, San Francisco, Santa Clara, Solano, Stanislaus, Stockton, Tulare and Ventura Counties.   

Compensation We Offer

  •  The initial compensation for this position ranges from $31.40-$38.61 per hour.
  •  Salary is dependent on commensurate experience above the minimum qualifications for the role and internal equity considerations.
  •  The salary may also vary if you reside in a different location than the location posted. 
  •  8% License Differential for LMFT, LCSW, LPCC, PsyD. and select Peer Certifications.
  •  5-7.5% Bilingual Differential for qualified positions

JOB SUMMARY

Works within the vision, mission and philosophy of the agency. Exhibit behaviors consistent with the Service Principles. Demonstrates and develops supportive/coaching relationship with patients and their caregivers to help them navigate the system to support their well-being.  This includes providing assistance (e.g., attending appointments, providing community linkages) and supporting education related to physical health, behavioral health, and social determinants of health. Supports whole-person care principles.

RESPONSIBILITIES AND DUTIES

  1. Demonstrates mastery of all System Navigator job descriptions, responsibilities, and duties. 
  2. Demonstrates an advanced ability to develop respectful interpersonal relationships and engage consumers in person centered care coordination. 
  3. Assists program leadership with modeling and coaching System Navigators on how to motivate and support customers with engaging in own care planning and goal setting. 
  4. Assists program leadership with onboarding new System Navigators by providing shadowing opportunities, one to one support to share developed best practices, and offering guidance around accessing community resources. 
  5. Utilize person-centered practices to complete assessments and care plans consistent with program needs and funding source expectations under supervision of licensed clinician. 
  6. Participates in and supports development of team meeting agendas, team building activities and ongoing program training. 
  7. Participates in and supports development of any program workgroups to support the development of System Navigator’s skills and competencies and customer and staff care needs.
  8. Models and coaches’ effective self-care and coping techniques to System Navigators. 
  9. Provides consultation support and advocates for consumer voice to be integrated into program practices and leadership decisions. 
  10. Assists program leadership with new documentation, training, or other process roll-outs to the team.

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. MINIMUM EDUCATION AND/OR EXPERIENCE

  • High school diploma or GED required.
  • Minimum of 2 years' experience working in related field (primary care, social and mental health, and public health)

HIGHLY DESIRABLE QUALIFICATIONS

  • Self-identified as a person with lived (or family) experience with mental illness and the mental health system.
  • Ability to identify recovery principles and explain their recovery process.

OTHER SPECIFIC REQUIREMENTS

  • Must possess a valid California driver's license and maintain an insurable driving record under the agency’s liability policy (if driving two or more times per week on company business) OR if driving is not required, demonstrated ability to use public transportation, if applicable.
  • National Provider Identifier (NPI) registered as a Mental Health Worker 
  • Must know or quickly acquire extensive knowledge of local community resources.

PHYSICAL REQUIREMENTS

While performing the duties of this job, the employee is frequently required to stand or sit. The employee is required to use hands to produce records and/or documentation in manual or electronic format. The employee must regularly lift and/or move up to 5 pounds and occasionally move or lift up to 10 pounds. Driving Privileges

  •   May be called upon to drive on agency business. Needs acceptable MVR and appropriate insurance coverage.
  •   Required to drive agency vehicle. Must have clean MVR, appropriate insurance coverage, and attend agency driver training.
