The Home Health Aide provides assistance with Hospice patients' activities of daily living and household services associated with their illness under the direction of the Hospice Registered Nurse Case Manager.

QUALIFICATIONS

License /Certification

Required:

Home Health Aide certificate obtained by: completion of a certified home health aide course, and certified by State Department of Health Services.

Certified Nursing Assistant certification

JOB RESPONSIBILITIES

Essential

Demonstrates effective communication and cares for patient and family as reflected in the Plan for Provision of Care.

Demonstrates competency in observing, reporting, and documenting patient status and the care or service provided to the patient.

Obtains and records patient's vital signs (temperature, pulse, and respirations; reports any changes in patient's condition to the Registered Nurse.

Demonstrates compliance with basic infection control procedures.

Demonstrates understanding of basic elements of body functioning and changes in body function(s) that must be reported to the Registered Nurse.

Promotes and maintains a clean, safe and healthy environment.

Demonstrates knowledge of emergency procedures and how to recognize emergencies.

Demonstrates and assists with appropriate and safe techniques in patient's personal hygiene and grooming including bed, sponge, tub or shower bath, skin and nail care, oral hygiene, hair shampoo and care; toileting and elimination

Demonstrates safe transfer techniques and ambulation of the patients as prescribed by appropriate clinical personnel.

Performs normal range of motion and positioning techniques with patients.

Promotes adequate nutrition and fluid intake.

Assists with household services which are essential to the patient's health care at home.

Records treatments and patient's progress; completes all written reports.

Gives emotional support to the patient and family.

Assists with the orientation of new employees.

Continually updates knowledge by attending in-services, and staff meetings. Provides evidence of completion of 12 in-service hours each calendar year.



Additional

Demonstrates the knowledge and skills necessary to provide care and services appropriate to the population served on the assigned unit or work area. Knowledgeable of growth and development for all patient/family cultural, linguistic, spiritual, gender, and age specific needs. Able to effectively communicate and care for patient and family as reflected in the Plan for Provision of Care.

Performs other duties as assigned.

Pay Range

$20.38 -$30.57
