Position Summary
The Intake Coordinator is responsible for reviewing, assessing, and evaluating case eligibility for CIPP/DCIPP meetings. This role requires strong organizational and communication skills to ensure the timely and efficient coordination of case reviews, scheduling, and documentation. The Intake Coordinator will work closely with internal teams and external partners to facilitate case management processes and improve service delivery for children and families.
Key Responsibilities
- Receive case notifications and provide CIPP/DCIPP Universal Referral Forms to Caseworkers for completion.
- Review and assess completed paperwork to determine case eligibility for CIPP/DCIPP meetings.
- Research and gather critical case information, including Child Protection reports, medical records, school reports, psychological evaluations, caregiver data, and court documents.
- Enter referrals into the CIPP/DCIPP scheduling system and distribute referral materials to facilitators.
- Coordinate and schedule CIPP/DCIPP meetings with identified participants, such as biological parents, children/youth, substitute caregivers, DCFS/POS Supervisors, Caseworkers, and Guardians Ad Litem.
- Provide caseworkers and supervisors with necessary documentation for CIPP/DCIPP meetings.
- Build and maintain effective working relationships with internal teams, caseworkers, supervisors, and external partners.
- Attend and participate in various regional meetings as needed.
- Assist in quality assurance and improvement efforts related to the CIPP/DCIPP process.
- Support case triage and emergency review planning with supervisors.
- Maintain accurate case records within the organization’s electronic system, ensuring compliance with all required guidelines.
- Generate status and administrative reports for internal teams and leadership.
Qualifications
- Required: High School Diploma and at least two years of experience in casework, case management, or a related field.
- Preferred: Bachelor’s Degree and two years of professional casework/case management experience.
- Strong organizational, communication, and computer skills.
- Experience in healthcare or Joint Commission-related safety standards is a plus.
- Ability to travel as needed to attend meetings and coordinate services.
