Preps, scans and/or performs quality review on patient charts entered into the optical record system for permanent retention.
Responsibilities:- Pick up inpatient and same day charts from all nursing units and maintains accountability for charts from all nursing units within 48 hours of discharge.
- Accurately prepares charts for scanning. Responsible for document entry via all scanning equipment and uploading from shared folders via the shared drive.
- Performs Indexing on all charts and transfer data to permanent medical record.
- Required skills must include a thorough working knowledge of chart content and chart order to enable assembly of a quality product of all patient type records.
- Must be able to identify scanner, printer and fax equipment problems and seek immediate resolution.
- Flexibility in scheduling may be necessary based on patient volume and patient care needs.
- Release patient information to nursing units and other healthcare facilities as appropriate for continuation of care.
- Reconciles Scanning/Indexing worklists in Expanse.
- Maintain office area to include stocking supplies, cleaning, keeping the area organized, making appropriate person aware of needed supplies and/or services, and working with others to ensure shift flow.

- High School diploma or equivalent.
- Typing and filing skills.
- Additional specialized courses or training in medical records preferred.
- Must have strong personal computer skills.
- Exhibits interpersonal skills and abilities to deal effectively with all levels of staff.
- Must be detailed oriented, self-motivated and have the ability to stay focused on tasks for extended periods of time.
- Must be flexible and have the ability to adapt to stressful situations and work schedules.
- Must be able to read, speak and write English.
