Must possess an advanced degree (i.e., PharmD), and have completed a post-graduate year one (PGY-1) residency (preferably in ambulatory care clinical pharmacy) or have equivalent ambulatory care or managed care clinical pharmacy experience; preferably with anticoagulation management experience if assigned to anticoagulation practice area. Licensed as a Pharmacist to practice in Massachusetts. Must meet or be eligible to meet all criteria for CDTM per Massachusetts Board of Pharmacy regulations. %3C/span%3EAmerican Heart Association Basic Life Support (BLS) required.
