Job Summary
Responsible for overseeing the daily operations of clinical care coordination and case management functions. Ensures the delivery of high-quality patient care by optimizing workflows, managing staff performance, and facilitating communication between care teams. Works to streamline processes, improve patient outcomes, and ensure compliance with hospital standards and regulations.
Qualifications
Education
- Bachelor's Degree Nursing required or
- Bachelor's Degree Healthcare Administration required or
- Bachelor's Degree Related Field of Study required

- Registered Nurse [RN - Massachusetts]
Experience
- Clinical experience in case management or care coordination, 3-5 years required
- Experience in a supervisory role, 1-2 years required

- Strong leadership and team management skills to effectively guide clinical staff.
