Social Worker-Hospice

As the Social Worker, you will provide medically related social services assessing individual needs/goals to maintain or enhance the psychosocial, mental, and emotional needs of our residents. Knowledge of end-of-life cares and needs. You will act as a liaison between the resident, family, and interdisciplinary team. Document all resident care plan information while following State and Federal codes. Additionally, you will perform an assessment of residents at admission, upon change in condition, and/or annually to identify and seek ways to support residents’ individual needs.
