RESPONSIBILITIES 

Including, but not limited to: 

  • The primary purpose of the Unit Coordinator is to support the nursing staff of the unit. 

  • Support CNA’s and audit documentation. 

  • Work with MDS to assure that documentation is completed accurately and in a timely manner. 

  • Provide clerical and non-clerical support to both staff and residents. 

  • Responsible for answering, coordinating, and managing phone calls to the unit. 

  • Coordinate all paperwork coming into the unit, including fax referrals and admissions. 

  • Prepare and maintain all patient medical records in a neat and orderly manner. 

  • Responsible for scheduling patient diagnostic tests, appointments, and consult upon direction of physician or Nurse 

  • Arrange resident escort and transport services. 

  • Maintain adequate supplies for the unit. 

  • Respond to residents' clinical needs. 

  • Assist with escorting residents within the facility. 

  • Assist with meal delivery and support on designated unit. 

  • Excellent organization, communication, and presentation skills 

  • Must possess integrity, initiative, and work well in a collaborative team environment 

  • Ability to work independently, problem solve, and make decisions as necessary 

  • Basic computer skills 

  • Additional duties as necessary 

  • Document residents' information accurately while ensuring compliance with HIPAA regulations. 

  • Collaborate with healthcare team members to deliver comprehensive resident care in our facilities.  

SKILLS

  • Current CNA  

  • Extreme organizational skills and follow through. 

  • Solid working knowledge of Word, Excel and other software programs  

  • Strong understanding of resident care principles and practices. 

  • Ability to provide compassionate support in hospice care settings. 

  • Proficient in lifting techniques to ensure the safety of both patients and staff. 

  • Familiarity with Skilled Nursing, Sub-acute, and Memory Care and the specific needs of residents. 

  • Understanding of HIPAA regulations to maintain resident confidentiality. 

  • Ability to work effectively in high-pressure situations during an emergency. 

  • Excellent communication skills to interact with residents, families, and other healthcare professionals. 

  • This Nursing Assistant role is essential for ensuring that residents receive the highest standard of care while promoting their dignity and comfort throughout their healthcare journey.
