Job Title: RN Home Care Agency Director

Location: Trinity at Home - Wilmington

Position Type: Full-time, Hybrid

Salary: Up to $90,000 per year

We are seeking a dynamic and experienced RN Home Care Agency Director to join our team in Wilmington. This role is crucial in maintaining the high standards required for our continued licensure and ensuring the effective delivery of our home care services.

 

Position Summary: The RN Home Care Agency Director is responsible for overseeing the daily operations of the home care agency, ensuring compliance with North Carolina Division of Health Service Regulation standards, and providing supervision and evaluation of program deliverables. This includes companion, sitter, respite, in-home aide, and nursing services.

 

Key Responsibilities:

  • Evaluate and present annual program evaluations based on feedback from clients, family caregivers, and referral sources.
  • Hire, supervise, and terminate staff in compliance with Title VII of the Civil Rights Act of 1964.
  • Develop and implement staff orientation and in-service training programs.
  • Coordinate staffing levels to meet client needs.
  • Supervise and conduct annual reviews of home care staff.
  • Implement agency policies and procedures.
  • Maintain accurate financial records and reports.
  • Process A/P, A/R, payroll, and billing procedures.
  • Operate within the approved budget.
  • Organize admission, discharge, supervision, and provision of services to clients.
  • Direct and monitor quality improvement activities.
  • Maintain ethical business and clinical practices.
  • Resolve client complaints and concerns.
  • Collaborate as the HIPAA privacy officer.
  • Ensure initial assessments and care plans are developed and reviewed quarterly.
  • Supervise and support home care workers.
  • Develop a backup plan for the Agency Director or RN supervisor's absence.
  • Conduct quarterly supervision of in-home aides.
  • Facilitate quarterly client record reviews.
  • Support on-call services as needed.
  • Validate skills of home care workers and complete skills validation checklists.

Qualifications:

  • Current valid RN license with the North Carolina Board of Nursing.
  • Minimum of 2 years’ experience in an administrative or supervisory capacity in a home care or health care setting.
  • Knowledge of home care and/or health care standards, policies, and reimbursement practices.
  • Ability to develop and maintain rapport with clients, families, and staff.
  • Excellent written and oral communication skills.
  • Strong problem-solving and decision-making abilities.
  • Willingness to travel as needed.

Working Conditions:

  • Ambulatory throughout all areas of the agency and clients’ homes.

Job Title: Trinity Grove Neighborhood Coordinator (Registered Nurse)

Location: Trinity Grove

Employment Type: Full-Time

Schedule: Monday through Friday, with on-call weekends

Pay: Base pay of $40 per hour

 

Position Summary: Trinity Grove is seeking a dedicated and dynamic Neighborhood Coordinator to join our team. The primary purpose of this role is to supervise the day-to-day activities within the assigned neighborhood, ensuring that the highest degree of quality care is maintained at all times. The Neighborhood Coordinator will work closely with various departments to establish a framework for person-centered resident care, in alignment with the philosophies and objectives of Trinity Grove.

 

Key Responsibilities:

  • Assist in directing the day-to-day functions of neighborhood activities, including planning services, programs, and activities.
  • Participate in the facility’s Quality Assurance and Performance Improvement program, attending committee meetings and preparing reports as requested.
  • Perform administrative duties such as completing forms, reports, evaluations, and investigations.
  • Provide leadership to personnel, including coaching, training, and mentoring new employees, developing consistent work assignments, and completing performance evaluations.
  • Conduct daily rounds, observations, and assessments.
  • Demonstrate knowledge of emergency care, disaster and evacuation policies and procedures, and safety programs.
  • Review resident records periodically to ensure completion, accuracy, and adequate reflection of the resident’s care and condition.
  • Consult with physicians in providing resident care and participate in resident care plans.
  • Meet with residents and/or family members as necessary, following up on grievances and ensuring all residents are treated with dignity, kindness, and respect.
  • Participate in the infection control program and ensure established safety guidelines are followed.
  • Honor resident choice, allowing residents to direct their own care and make personal choices regarding daily schedules and activities.
  • Reflect Trinity Grove’s mission, vision, values, and customer service standards in daily interactions.

Qualifications:

  • Registered Nursing degree from an accredited college or university, or a graduate of an approved RN program.
  • Licensed registered nurse in the state of North Carolina.
  • CPR certified or ability to become CPR certified.
  • One to two years of supervisory experience preferred.
  • Strong communication skills and ability to maintain confidentiality.
  • Computer skills preferred; willingness to learn electronic documentation.

Working Conditions:

  • Physical exertion involving pushing, pulling, and lifting up to 35 pounds; occasionally lifting weights up to 50 pounds.
  • Prolonged standing and/or walking during which time objects are transported.
  • Occasional bending, squatting, or reaching repeatedly.
  • Ability to assist in the repositioning and transferring of individuals of varying weights following established safe lifting procedures.
