This is a full time, benefits eligible position working 80hrs biweekly, includes working every other weekend and rotating holidays.
Shifts Available
- 1st shift (6:00am - 2:00pm
- 2nd shift (2:00pm - 10:00pm
- 3rd shift (10:00pm - 6:00am)

Primary Purpose: The Trach and Ventilation Nurse provides specialized nursing care for individuals with intellectual and developmental disabilities as part of an interdisciplinary healthcare team. The position entails continuous assessment, monitoring and management of respiratory needs and ensuring an integration of their complex medical and developmental needs into a person-centered care plan.
Essential Job Functions:
- Deliver skilled nursing care to ICF residents with tracheostomies and or mechanical ventilation needs in accordance with physician orders and regulatory compliance.
- Perform daily tracheostomy care, suctioning, stoma assessments, and tube changes as per protocol.
- Monitor ventilator settings and alarms and promptly respond to respiratory needs.
- Administer Medication and Treatments as ordered by physician in accordance with the Medication Administration Policy.
- Provide informed care that respects the individuals preferred communication style.
- Provide support and education to Direct Support Professionals and other team members on care protocols, safety measures, and emergency protocols.
- Document nursing assessments, medications, treatments, in electronic and paper form according to ICF regulations.
- Collaborate with interdisciplinary team members including physicians, respiratory therapists, behavioral support, speech, occupational, and physical therapies, direct care, quality and residential management.
- Provide support with respiratory weaning and decannulation processes when ordered.
Qualifications:
- Licensed RN or LPN in Pennsylvania
- 2 years of nursing experience.
- Ventilator dependent and tracheostomy experience preferred.
Knowledge, Skills & Abilities:
- Proficiency in respiratory care
- Ability to adapt care to accommodate cognitive and sensory needs.
- Strong clinical judgment and emergency response skills.
- Effective communication with interdisciplinary team.
- Knowledge of ICF regulatory standards
