Position Summary
As a Physical Therapist, you will assess residents, develop personalized treatment plans, and implement therapy programs to restore mobility, improve function, and promote independence. You’ll play an important role in improving quality of life and supporting the rehabilitation goals of our residents.

Key Responsibilities
• Evaluate patients’ physical conditions and establish individualized treatment plans.
• Deliver skilled physical therapy services to enhance mobility, balance, and strength.
• Monitor patient progress and adjust treatment plans as needed.
• Educate residents, families, and caregivers on exercise programs and safety techniques.
• Document all services in accordance with facility, state, and federal requirements.
• Collaborate with the interdisciplinary team to ensure comprehensive care.
• Maintain compliance with all company policies and procedures.

Qualifications
• Active Texas Physical Therapist license
• In good standing with all licensing boards and agencies
• Experience in Skilled Nursing Facilities (SNF) or long-term care preferred
• Strong communication and interpersonal skills
• Familiarity with Net Health documentation system a plus
