Program and Role Summary:
Wayfinder's Post-Adoption and Permanency Services Program provides ongoing support to families who have adopted through foster care, private adoption, and kinship placements. We help families navigate the unique challenges of adoption and post-permanency by connecting them with essential resources, providing trauma-informed support, and offering immediate assistance during times of crisis.
The primary focus of the Social Worker II is to provide preventive, supportive case management services specifically to pre/post adoptive families. These services will include short-term interventions based on the individualized needs of the family, primarily concerning adoption-related issues. The Social Worker II will assess family needs and assist with connection/access to adoption competent therapeutic services and community linkages.
Primary Responsibilities:
- Case management of pre/post adoptive families and assessments to complete a family support plan.
- Assess family needs and assist families with appropriate community linkages, advocacy support, and skill building.
- Provide families with strategies to assist with increasing attachment in their home and in learning a variety of techniques to respond to challenging behaviors.
- Provide education and information to assist families in understanding the family situation and learning strategies in parenting.

Qualifications
Education and Experience:
- Master’s degree in social work or related field from an accredited college required.
- Experience working with pre/post adoptive families is preferred.
- Ability to travel to family homes and meeting within Placer County, required.
- Valid CA Driver License with a minimum of three (3) years good driving history and must be approved by the agency’s insurance carrier to drive agency vehicles.
