QUALIFICATIONS
Education
- M.S. in social work or B.S. with M.S. supervision
Experience/Skills
- Prefer a minimum of one year of social work experience in a health care setting
- Knowledge of the job description for Medical Social Worker
- Ability to perform all the essential functions of the job outlined below without creating a direct threat to the safety of oneself or others.
- Involves making home visits to clients and families as assigned by the director.
- Communication and interaction with others.

Essential Functions
- Evaluates the psychosocial situation of the client and family as it relates to the client’s illness, his/her need for care, his/her response to treatment and his/her adjustment to the care provided.
- Develops and implements a social work treatment plan.
- Takes appropriate action to obtain case work services to assist the client and family with needs.
- Assesses the relationship of the client’s medical and nursing requirements to the client’s home situation, his/her financial resources, and community resources available to him/her.
- Consults with other agencies for appropriate information, serves as Social Service Liaison between community agencies, social service departments, including discharge planners of Sullivan County Community Hospital.
- Acts as a consultant to staff on social and financial resources for active and prospective clients.
- Assists in the development and implementation of projects and/or research relative to the social work program.
- Maintains responsibility to complete mandatory education as assigned.
- Provides visit notes that easily reflect the covered care being provided to the client.
- Demonstrates skill in conflict resolution.
- Responsible for maintaining consistent and concise lines of authority.
- Initiates consultation as issues and concerns arise.
- Provides accurate information of the client visits for billing purposes.
- Maintains confidentiality of all client information.
- Participates in departmental committee as requested.
- Accepts responsibility under adverse working conditions, within limits of safety, such as in inclement weather conditions, and substandard living conditions of clients.
- Performs other related duties and activities as may be requested.
- You must attend annually at least 25% of monthly departmental staff meetings and review/initial meeting minutes not attended.
- Participates in discharge planning and in-service programs.
- Participates in on-call to ensure patient access to on-going care.
