Job Summary:
The Housing Specialist - Subject Matter Expert (SME) will be responsible for the organization's housing navigation strategy, within the context of our initiative to identify and promote community-based housing solutions for our members. This role is responsible for designing and implementing a scalable framework to support housing access for all Members, with a particular focus on non-certified housing settings. The SME will bring deep, hands-on experience in housing systems, including housing subsidies, funding mechanisms, and navigation of complex housing landscapes. Certification as a Housing Navigator is required.
Rather than focusing on individual cases, the SME will develop tools, guidance, and best practices to empower Care Managers across the organization. This includes leading a cross-departmental Community of Practice dedicated to housing, fostering collaboration, and driving continuous improvement. The SME will also partner with Learning & Development to ensure Care Managers are equipped with the specialized knowledge needed to advocate effectively for Members and navigate housing systems with confidence.
Essential Duties and Responsibilities:

Educate and train the Care Management team on housing resources to ensure people supported are afforded opportunities for safe, accessible and affordable housing.

Educate Care Management team on housing options, funding models, and how to navigate the housing development process.

Assist Care Management, when necessary, in developing a housing related resource directory.

Assist in developing, implementing, and monitoring Policies and SOPs related to housing opportunities, initiatives, and resources.

Timely and professional communication with all applicable parties to maintain and resolve issues or concerns.

Acts as a Subject Matter Expert (SME) to work in partnership with the Institute for Learning and Development (ILD) on trainings and seminars for Care Management staff related to housing opportunities, initiatives, and resources.

In partnership with ILD conduct “office hours” for Care Management staff to be able to ask questions related to housing opportunities, initiatives, and resources.

Forms and maintains relationships with housing organizations across the CCOs geography to harness opportunities for members.

Works in Partnership with the Provider Relations teams to further develop relationships with provider partners as needed.

Collaborates with Member Relations and Marketing team to educate families and members on various housing options.

Facilitates a Community of Practice related to Housing Navigation.

Follows all mandated reporting requirements.

Adheres to all company policies and procedures.
Qualifications
Education and Experience:

Bachelor’s degree in health and human services and at least two years’ experience working with people with IDD.

Certification as a Housing Navigator through a reputable organization such as the NY Alliance for Inclusion and Innovation (NYAII), the Council on Developmental Disabilities, or Homeless and Housing Resource Center (HHRC) is required.
