Description
The main role of the Waiver Intake Specialist is to guide prospective Participants referrals through the waiver application process, affording them the opportunity to receive Home and Community Bases services through Community Health Choices or Office of Developmental Programs. The following details the day-to-day processes, which vary depending on the status of the referral:
- Ensure referrals understand the services offered and help them navigate any challenges.
- Keep detailed records of referral interactions and status updates.
- Conduct follow-up calls and meetings with referrals to ensure they complete the approval process.
- Call referrals to provide reminders and follow up on completing their required assessments.
- Provide regular updates to the Business Development team on referral status and any issues encountered.
- Contact Primary Care Physicians (PCP) offices to obtain necessary physician-signed forms.
- Follow up with County Area Agencies on Aging (AAA) to schedule Functional Eligibility Determination (FED) assessments for referrals.
- Track and complete required $500 transactions with proper documentation for County Assistance Office (CAO) review.
- Organize and categorize referral sources by rehab/hospital locations to streamline outreach efforts.
- Maintain and update spreadsheets to track referral progress and status
- Ensure that every referral signs the required Medicaid enrollment form and follow up as needed.
- Verify each referrals’ profile is accurately tracked and identify any missing information so it can be addressed in the next follow-up call.
- Other duties as assigned.
