In this unique and fast-paced role, you’ll help shape the patient experience before the visit even begins — coordinating, reviewing, and implementing pre-visit planning activities for all of PCHC’s adult primary care practices. You’ll serve as a trusted bridge between providers, care teams, and patients, helping everything run seamlessly. Schedule: Full-time, Monday-Friday, Schedule TBD (Hybrid remote position, with some travel expected to PCHC clinics. Candidate must be eligible per PCHC’s Telecommuting policy.)
Highlights of the position:
- Acts as a key liaison to the organization by demonstrating excellent communication skills, initiative, self-direction, confidentiality, discretion and mission-focus while communicating with patients and collaborating with providers and clinical staff.
- Is responsible for Organization Provider Desktop Management: reviews, signs and flowsheets documents, lab results and cancer screenings for all primary care PCHC providers.
- Conducts pre-visit planning for Penobscot Community Health Center utilizing various EMRs to focus on patients with chronic conditions to help improve health outcomes for patients with complex needs.
- Utilizes HIN analytics in conjunction with various EMRs to run reports, compile data, and prepare outcome analysis.
- Maintains accurate and timely documentation.
- Is flexible in supporting the needs of the clinic, stepping in to perform other functions within scope as requested by Supervisor.
- Is innovative and creative, constantly looking for unique ways to improve overall performance and the patient experience.
Education and Experience:
- High School diploma or equivalent
- Evidence of Medical Assistant training including all the essential job functions, knowledge, skills and abilities listed here in this job description, or MA Certification.
- 3+ years Medical Assistant Experience required
- Will have up to date training and certification in Healthcare Provider Basic Life Support Issued by the American Heart Association, American Red Cross, or American Health and Safety Institute. Must obtain within 2 weeks of hire.
- Initial and maintained Certification is highly encouraged and supported.
