Mental Health Nurse Practitioner Functional Medicine & Primary Care

With over 18 clinics across Oregon and growing, we’re seeking a passionate Mental Health Nurse Practitioner to join our primary care team and make a difference through holistic, functional, and patient-centered care.
This role is a hybrid role working in Southern Oregon
Compensation: Guaranteed salary with earning potential of $215,000+

RESPONSIBILITIES AND DUTIES:
- Provide comprehensive psychiatric evaluations and ongoing medication management for patients across the lifespan.
- Diagnose and treat a range of mental health conditions, including anxiety, depression, mood disorders, and ADHD.
- Develop individualized treatment plans that may include medication, therapy recommendations, and lifestyle interventions.
- Collaborate with primary care providers, health coaches, and behavioral health professionals to support integrated, whole-person care.
- Educate and empower patients on mental health, wellness strategies, and preventive care.
- Maintain accurate and timely documentation in the EHR (NextGen).
- Participate in team meetings, clinical collaboration, and quality improvement initiatives.
REQUIRED QUALIFICATIONS:
- Active Oregon license as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) or eligibility to obtain.
- Board certification as a PMHNP is required.
- Minimum of 2 years of clinical experience preferred
- Strong interest in functional, lifestyle, and integrative approaches to mental health.
- Excellent communication and interpersonal skills with a patient-centered mindset.
Ability to work collaboratively in a multidisciplinary team environment. - Proficiency in using electronic health records (NextGen experience preferred).
