About Care Resources:
- Care Resources is a community-based program committed to fostering healthy, independent living for individuals aged 55 and older.

Job Duties and Daily Tasks:
- Schedule outside provider appointments for the participants
- Schedule onsite specialty provider appointments
- Create referrals
- Request medical records and authorize services

Education and Experience:
- Position requires a high school diploma or equivalent.
- Our ideal candidate will have experience with electronic medical record computer systems and scheduling
- Minimum of 2 years of experience in general office processes; medical terminology knowledge is a plus
- Must have valid driver’s license and insurance and reliable transportation to make home visits
- Prefer 1 year of experience working with the frail or elderly population
Hours and Schedule:
- Full-Time: Monday - Friday onsite at our facility located at 4150 Kalamazoo Ave SE, Kentwood, MI 49508
- 8 am - 4:30 pm
- Occasional travel to participant homes during work hours required
