Description

Position Summary:

This position is responsible for linkage services in support of clients with substance use and/or dual diagnosis disorders currently residing in one of TCN’s recovery houses. This position may also be responsible for covering SUD Outpatient groups as needed. This position will be participating in one PHP group and/or 1 SUD IOP/OP group per quarter for training purposes to remain current with group curriculum, unless they have provided group coverage within the same quarter.

 

Key Responsibilities:

  • Provides individual case management, group counseling, and/or individual counseling under clinical supervision; and maintains a residential environment which promotes positive resident growth and the achievement of treatment goals in a manner that meets quality requirements set by the Agency, and that is within the limits of services permitted by position and scope of practice:
    • Assists in designing and participating in resident activities.
    • Assists in identifying resident treatment needs and providing appropriate interventions and interactions as directed by supervisor.
    • Facilitates educational/therapeutic groups and other therapeutic activities as needed.
    • Reinforces residents’ orientation to house rules, safety procedures, and household environment.
    • Utilize the residential plans of care for each resident.
    • Monitors the provision of self-assisted medication.
    • Monitors each client’s general health status.
    • Ensures that the facilities are always pleasant, and that they meet the cleanliness standards of Ohio MHAS and TCN standards.
    • Respects and protects the clients’ rights.
    • Complete all safety procedures and drills in strict accordance with Agency schedules and standards.
    • Transport clients as needed, using company vehicles and/or personal vehicles, pending approval from TCN’s drive-check agency.
    • Accompany clients to various locations, ensuring continuous monitoring throughout their activities. This includes accompanying clients into stores, appointments, waiting rooms, support meetings, and other relevant locations.
    • Thoroughly reads, comprehends, and consistently enforces all client rules without exception. Escalates any violations to supervisor.
  • Reports daily activities and other pertinent information.
  • Documents of client progress and activities in residential charts and in daily shift reports.
  • Assists in the preparation of facility summary reports.
  • Engages in ongoing communication with community support personnel, supervisors, and other pertinent clinicians to relay information and updates.
  • Maintains 100% accuracy in documentation and summary reports.
  • Completes documentation according to TCN standards.
  • Provides only those services authorized by Utilization Management and provides services within the authorized frequency and duration of services.
  • Individualized Service Plan, Progress Notes and other documentation clearly identifies consumer’s input in services by having consumer’s signature when clinically appropriate.
  • Completes progress notes at the end of each session and submit all documentation within 24 hours of the service provided.
  • Develops and maintains quality, accurate clinical charts for all clients served.
  • Handles crisis situations, as needed, calmly and effectively.
  • Mediates conflict among residents as needed, promoting a respectful and peaceful living environment.
  • Maintains open communication with clients, staff, and other stakeholders.
  • Demonstrates a level of alertness, approachability, availability, and readiness.
  • Adheres to all organizational and departmental policies, complying with all behavioral and ethical expectations.
  • Complete all agency-required training, including client rights and grievances, safety training, infection control, AEGIS training, etc.
  • Demonstrates and supports the company core values TCN C.A.R.E.S. “Commitment, Accountability, Respect, Empathy, Service.”
  • Develops and maintains positive collaboration with other social service agencies serving clients. Maintains knowledge and familiarity with referral sources within the community.
  • Serves as liaison between the criminal justice system and treatment services in the jails.
  • All other recovery housing and/or clinical duties as assigned.

 


Qualifications


 Minimum Qualifications:

  • Education: Associate degree in human/social service field.
  • Experience: One year in the field of alcohol and substance abuse residential treatment

preferred.

  • Certification: CDCA or above required, SWA or above preferred; CPR; First Aid; AEGIS (De-escalation) training; Medication Monitoring.
  • Other: Valid Ohio driver’s license; proof of automobile insurance; meet ODADAS licensure and certification requirements; demonstrated competencies for working with persons with substance use and/or mental health disorders.

Knowledge, Skills, & Abilities:

Knowledge of:

  • Safety rules and procedures, Agency policies and procedures, local area and resources, daily living tasks and skills, documentation requirements, crisis management, empathy and compassion, and ethical conduct.

Skills:

  • Capacity to perform duties effectively under potential emotional stress and conflict situations.
  • Proficiency in utilizing basic computer skills and office equipment for documentation and communication purposes.
  • Practice of a client-focused approach, demonstrating active listening skills, reading comprehension, critical thinking, and leadership abilities.
  • excellent interpersonal skills, including directing and demonstrating activities of daily living, i.e., cooking, cleaning, laundry, etc.
  • operating basic household appliances and tools.

Abilities:

  • Willingness and physical capability to engage in activities are essential to meet the fundamental needs of individuals served. This includes occasional assistance in relocating individuals to new living situations, aiding with shopping, and instructing daily living skills, as necessary.
  • Lifting to 25 pounds, standing, bending, stooping, writing legibly or using computer, ability to read, and communicate information clearly, ability to recognize unusual or threatening conditions, and ability to remain patient and understanding when working with individuals in various stages of recovery.
