Full-time, 35 hours/week
A hybrid schedule of three days remote and two days in the office may be possible after training.
Will provide the medical/nursing component to a multidisciplinary team composed of licensed practical nurses, registered nurses and case managers/social workers. The team facilitates the screening of new caregivers, the Adult Foster Care (AFC) settings and members, the matching of members and families, and the on-going services to program members.
Under the direction and supervision of an AFC Registered Nurse, the Licensed Practical Nurse may perform the following duties.
- Provide orientation and ongoing healthcare training and education to assigned program members and caregivers.
- Develop, implement, and monitor members' Plan of Care (POC).
- Maintain good working relationships and open communications with doctors and other professionals in the community in order to monitor health statuses of each member.
- Assist Adult Foster Care in the development and implementation of outreach and marketing plans.
- Maintain strong working knowledge of AFC funding sources, including MassHealth, SCO, One Cares, and Private Pay and understands how each funding source directs the scope/manner in which AFC Program services are delivered.
- Maintain case files/electronic files by completing all mandated paperwork in a timely fashion according to required documentation standards.
- Participate in required quality performance initiatives within program area.
- Complete PCP Order Form to obtain MassHealth or SCO/One Care prior approval.
- Review client's plan of care and health status in conjunction with the primary care physician annually and semi-annually for accuracy and updates.
- Provide telephone support when needed to on-call staff for AFC Members and Caregivers.
- Participate in after-hours telephone coverage to address emergency situations.
- Must be able to climb multiple flights of stairs
Education and Experience:
- Licensed Practical Nurse (LPN) must be licensed by the Massachusetts Board of Registration in Nursing and in good standing with the Board.
- At least one year of experience providing care to elderly/disabled individuals.
Public Service Loan Forgiveness (PSLF)
