ABOUT US:

Community Health is a multi-siteFQHC serving 80% of the primary care needs in our community (central Vermont).Our mission is to service our patients with quality, efficiency, accessibilityand equity. As the largest FQHC in northern New England, we have a largefootprint that provides us with the opportunity to influence the evolving,local, state and regional health care environment.

Community Health Center sites arecertified Level 111 Medical Homes, with a highly developed and supportive panelmanagement and case management system in place. Community Health also has arobust integrated behavioral health program employing Psychiatrists, BehavioralHealth Nurse Practitioners and Licensed Clinical Social Workers embedded withinits primary care sites. We have a fully integrated electronic medical recordand practice management system (Medent) with robust interfaces with our localhospital. 


POSITION SUMMARY:

Community Health is seekingan Express Care Physician to treat urgent care patients at our clinic inCastleton, Vermont.  Physiciancandidates are expected to be knowledgeable and skilled in the delivery ofquality care. Responsibilities include, but are not limited to:  

  • Provide appropriate patient follow-up and communicate effectively with patients.  
  • Refer patients to primary and specialty care as necessary.  
  • Provide clinical supervision to mid-level providers and nursing staff.  
  • Participate in quality assurance activities.  
  • Procedural skills, e.g. suturing, I&D, reading EKG and x-ray, etc.
  • Participate in State and National disease collaborative efforts. 

 

KNOWLEDGE, EDUCATION AND SKILL REQUIREMENTS:  

  • Previous Experience in Urgent Care or Emergency Room Services preferred.
  • Graduate of an accredited Acute Care Nurse Practitioner or Physician Assistant Program.
  • Licensed for Nurse Practitioner or Physician Assistant license in the state of Vermont.
  • Quality-focused team player.
  • Excellent organizational, time management and problem-solving skills.
  • Ability to function effectively as part of a multi-disciplinary team.
  • Effective oral and written communication skills.
  • Commitment to providing exceptional patient care.
